PASSPORT
Each student must have a passport. The passport must be valid throughout the entire stay. Students can apply for a U. S. passport, good for ten years, at any U. S. passport agency or at any post office which accepts passport applications. You can find out where you can apply for a passport by going to http://iafdb.travel.state.gov/ and entering a zip code or a city.

Steps in the Process:
1. Get the application form. You can get the application from the offices listed or passport forms are available on-line at http://travel.state.gov/passport/forms/forms_847.html. Do NOT sign the form until you are at the application office--they need to see you sign it.
2. Obtain documentation: As part of the application, you will need to submit a certified birth certificate (certification is usually indicated by a raised or embossed seal). Students without a certified birth certificate should contact the appropriate records office of the county or city in which they were born. If any student is uncertain of the exact jurisdiction under which he or she was born, the post office has a "Passport Services" form which describes the process for obtaining the birth certificate. You will need to apply in person You will need to present a government picture identification card (for example, a driver's license).
3. Have a passport picture taken: You will need to bring in two passport pictures. Most application sites do NOT have this service available.
4. FEES: There is an $97.00 fee for the application. A check for $67 will be made out the U.S. Department of State and another one for $30 will be written to the office to which you apply.
5. It takes 12 to 16 weeks to obtain a passport; APPLY
EARLY! Expedited
service is available for an extra fee (with appropriate documentation,
for example an airline ticket or an official trip schedule--see http://travel.state.gov/passport/processing/processing_1740.html).
LOST PASSPORT OR AIRLINE TICKET
If you lose your passport or airline ticket before you leave, you are fully responsible for any additional charges stemming from the loss.
Airline ticket: You are responsible for contacting the travel agent and paying for another ticket. You may then apply directly to the airline carrier for full reimbursement which can take 6 months to a year to process.
Passport: You must reapply for a new passport at the going rate, requesting a special Expedite Review which costs an additional $60.00. You must also attach a letter explaining what happened, why you need a new passport and your departure date.
